Register Nurse(RN)

•Conduct comprehensive patient assessments

• Monitor vital signs and symptoms
•Administer medications and treatments as prescribed
•Perform wound care and basic medical procedures

•Assist with bathing, grooming, and hygiene
•Support with mobility and transfers
•Prepare nutritious meals
•Perform light housekeeping tasks

Direct Support Professional (DSP) DD

•Provide direct support with hygiene and personal care
•Assist with medications (if trained)
•Ensure safety and well-being
•Document services per ISP
•Household management (meals, cleaning)
•Community transportation and activities
•Report changes/incidents promptly
•This aligns with roles used by Ohio provider agencies.
